When is an ERV required by code?

Where ventilation rates and climate make recovery mandatory or economically required. ASHRAE 90.1 sets minimum enthalpy recovery ratios of 50 or 60 percent depending on conditions, and EU Regulation 1253/2014 imposes its own floor.

Why does effectiveness vary so much?

Because effectiveness is not a constant. It is a function of the operating point: airflow ratio, temperature and humidity difference. A single catalogue effectiveness figure describes one condition, not the year your project will actually run.

Wheel or plate, and is an ERV the same as an HRV?

A wheel transfers both heat and moisture, a plate typically transfers heat only. An HRV recovers sensible heat alone, an ERV recovers total enthalpy. Which is right depends on your humidity load, not on a general preference.

Does the tool show the energy it costs to run?

Yes. Recovery is reported alongside the parasitic fan power needed to achieve it, because a recovery figure quoted without its pressure-drop penalty overstates the benefit.

How is frost control handled?

As an energy debit across the winter hours, not a footnote. Preheat or bypass strategies consume energy in exactly the conditions where recovery looks best, so the annual figure is only defensible if that debit is counted.
